Health Assessments done at RocklandMD Medical Clinic are based on the latest recommendations on preventive health exams.

A complete and personalized evaluation is achieved through a consultation with the physician and the blood tests. If needed, follow up care is provided.

RocklandMD Assessments type I and II

Type I 

Consultation with the nurse:

  • Creation of an electronic medical record
  • Completion of an initial healthcare questionnaire

Laboratory tests: 

  • Blood type
  • Complete Blood Count
  • Biochemistry and liver profile
  • Lipid profil
  • Thyroid profile (TSH)
  • Urine analysis
  • PSA (Prostate Specific Antigen) in men of 55 and over (after discussion with the client)
  • Thin prep test for women (if needed)
  • Fecal occult blood test every 2 years in men and women 50 and over

Medical evaluation:

  • Medical and family history
  • Lifestyle assessment: tobacco use, physical activity, alcohol consumption, allergies, medication
  • Complete physical exam including gynaecological examination for women 

Cardiorespiratory evaluation:

  • Electrocardiogram (EKG)  (if needed)

Additional tests:

  • Mammogram for women 50 and over (every two years)
  • Chest X-ray (if needed)
  • Osteodensitometry (if needed)
  • Colonoscopy for men and women (50 and over, if needed) (Additional fees apply)

Results:

  • Test results and assessment findings are given and explained to the patient by the doctor during the follow up visit.


Type II

Consultation with nurse:

  • Creation of an electronic medical record
  • Completion of an initial healthcare questionnaire

  • Complete blood count
  • Blood type
  • Biochemistry and liver profile
  • Lipid profile
  • Thyroid profile (TSH)
  • Vitamine D and calcium
  • Urine analysis
  • PSA (Prostate Specific Antigen) in men of 55 and over (after discussion with the client)
  • Thin prep test for women (if needed)
  • Fecal occult blood test every 2 years in men and women 50 and over

Medical evaluation:

  • Medical and family history
  • Lifestyle assessment: tobacco use, physical activity, alcohol consumption, allergies, medication
  • Complete physical exam including gynaecological examination for women 

Cardiorespiratory evaluation:

  • Electrocardiogram (EKG) (if needed)

Nutritional evaluation:

  • A 30 minute session with the dietitian-nutritionist to evaluate your eating habits in order to outline if specific dietary changes are needed along with the appropriate recommendations to help you improve your diet.

  • Mammogram for women 50 and over (every two years)
  • Chest X-ray (if needed)
  • Osteodensitometry (if needed)
  • Colonoscopy for men and women 50 and over, if needed (additional fees apply)

  • Test results and assessment findings are given and explained to the patient by the doctor during the follow up visit.

 

General information

The first visit:
The nurse welcomes the client and outlines the different steps of the health assessment process. The client’s computerized medical record is created and his/ health information is collected. Blood tests are performed.

After the initial consultation with the nurse, the client has a light snack. He than meets with the physician for the medical checkup. During the Health Assessment type II, an exercise stress test is done under the supervision of the physician.

Follow up visit:
During the follow up visit, test results and assessment findings are explained and client questions are addressed

 

Premium Annual Assessment

Additional consultation visits: 

  • Three (3) medical consultations that can be taken at any time (at the patients’ request) throughout the year following the initial visit.
  • All fees associated with other services (blood work, EKG and other) rendered during these visits are not included.
  • The consultations are transferable to your spouse/husband and/or your children over the age of 14 for minor emergencies.
